Families play a critical role in the continued success and growth of Team Southwest. They are a constant source of support and encouragement for their athlete. Special Olympics makes a remarkable difference in the athletes' lives by boosting self-esteem so they can become an intricate part of their community whether at home, at school, or in the workplace.
In order for your athlete to compete in any of our
tremendous events, they will need two things. First, a
signed permission form that is sent home through school or
can be requested by contacting our Head Coach and Special
Olympic Coordinator, Charlie Rowe at
708-259-4322 or firstname.lastname@example.org.
Second, they will need a "Medical Application for
Participation" completed by their physician. These forms are
valid for two years from the exam date. If you are unsure if
your athlete has an up-to-date form, please contact Charlie
Rowe. These forms can also be filled out and completed by a
doctor at the MedFest event in October.
